All forms must be submitted by noon on Friday, June 16, through this form or as a printed copy to Derek Henze at Edward Jones, 691 S. Janesville Street, Milton, WI 53563 (Next to Piggly Wiggly/Anytime Fitness) or submitted by email to derek.henze@edwardjones.com.

• Maintain forward motion and 50’ – 75’ intervals at all times.
• If your group or individual does performances, you are limited to 3 minutes at each stop.
• Any group or individual distributing candy or other favors which may entice children into the street, must do so with individuals walking next to the curb. Absolutely no throwing of candy or favors from vehicles will be allowed.
• Units are not to display price tags, “for sale” signs, or sales pitches. An appropriate sign of “who you are” will be acceptable.
• Motorized vehicles may not exceed speeds of 15 mph. Any motorized vehicle must be driven by a licensed driver. No exceptions.
• Absolutely no spraying of water, silly string, etc. on spectators.
• Political literature may only be handed out to individuals of legal voting age.
• Horses and other large animals will be placed at the end of the parade. In the interest of animal safety and to comply with requirements from the Wisconsin Department of Agriculture, Trade and Consumer Protection, parade entries with horses will be required to show documentation of the current Coggins test during parade line up on July 4th in order to participate in the parade. If proof of the Coggins test is not provided, the entrant will be ineligible to participate in the parade.

By submitting this form and electronically signing below I agree to follow the rules of the City of Milton and this Parade. In consideration for my rights to participate in this parade, I hereby release the Milton-Milton Township Fire Department, City of Milton, Town of Milton, the Open Doors Foundation, all sponsors, and all Parade Officials from any liability of personal injury incurred by me or my group participating in the July 4, 2023 Parade resulting from the negligence or intentional misconduct of myself or my group.
